Verrucous Cyst

HPV infection of the lining of an epidermoid, or rarely a tricholemmal, cyst can lead to a verrucous cyst. Verrucous cysts occur most commonly on the face, upper trunk, and plantar surface and may harbor a number of different HPV types, including those associated with epidermodysplasia verruciformis. In verrucous plantar cysts, HPV 60 has been the most commonly reported HPV type. Verrucous cysts have also been observed in association with proliferating epidermoid cysts and have been hypothesized to be a possible precursor.

Pathology

In addition to features of an epidermoid or tricholemmal cyst, these cysts have a lining with verrucous features including acanthosis, papillomatosis, focal hypergranulosis with coarse keratohyalin granules, and occasionally koilocytes.

Treatment

Excision is curative.
