CANNABINOIDS
There has been increasing interest in cannabinoids, the group of compounds that act on cannabinoid receptors and are often associated with Cannabis sativa or Cannabis indica. Within the field of dermatology, these compounds are of interest due to their potential antipruritic, anti-inflammatory, and anti-nociceptive properties, all of which do not require any psychoactive aspects of the plant. While there is promise for both topical and oral application of a range of compounds including cannabidiol (CBD), N-palmitoylethanolamide (PEA), and even whole plant extracts, there is surprisingly little clinical evidence for any of these, let alone approved formulations that have been tested in a rigorous fashion. In fact, a recent review identified only 8 clinical studies of cannabinoids for dermatologic conditions, with 7 focused on atopic dermatitis, eczema, or pruritus. Of these, 6 reported improvement based upon at least some metrics, while in the second largest study of this group, a topical PEA-containing product led to no significant differences in chronic pruritus.
There is a need for high-quality trials to evaluate efficacy and safety of particular products, as it is simply not appropriate to generalize results about an ingredient or individual compound given all the complex variables involved: formulation, route of administration, dosage, and frequency of use.
